Yes, but only in two places. CoolSculpting works under the chin and along the underside of the jawline, where a soft pocket of fat can be drawn into the applicator. It does not treat the cheeks, and it cannot lift jowls caused by loose skin. CoolSculpting Elite freezes fat cells so the body clears them over the following weeks, which is why it only works where there is a pinchable layer of fat to freeze.

The first area is the space under the chin, the fullness most people call a double chin. We cover that area in detail in our post on whether CoolSculpting works for a double chin.
The second area is the underside of the jaw on each side, from the chin back toward the ear. Fat here softens the line between the jaw and the neck, so the face can look rounder from the side even at a healthy weight. The same small applicator used under the chin fits along this edge of the jaw. Treating both areas in one plan gives a cleaner profile from the front and from the side.
A single treatment reduces fat in the treated area by about 20 to 25 percent. Many patients with fullness along the jaw see the best result after two sessions spaced about a month apart, and some need a third. Each session on the chin or jaw takes about 35 minutes, and most patients go straight back to work afterward.
Results build slowly. Most patients notice a change three to four weeks after the first session, and the final result shows at around three months. Fat cells that are frozen and cleared do not come back, so the result lasts as long as your weight stays stable.
Jowls form when the skin and soft tissue along the jaw lose firmness and slide down past the jawline. That is a laxity problem. Freezing fat does not tighten skin, so CoolSculpting on its own will not lift a jowl. Our post on whether CoolSculpting tightens skin explains why.
A quick way to tell the difference: gently pinch the area under your jaw. A thick, soft roll that you can hold between two fingers is usually fat. Thin skin that folds easily and sits lower when you look down is usually laxity. Many people have a little of both, and losing weight rarely lifts a jowl that comes from loose skin.
For laxity, we look at treatments that build collagen and firm the tissue. Sofwave uses ultrasound energy to lift and firm skin along the jaw and neck. EMFACE pairs radiofrequency with muscle stimulation to tone and define the lower face without needles. XERF is another radiofrequency option for skin that has started to sag. When a small fat pad also sits under the jowl, CoolSculpting can reduce that fat while one of these treatments handles the lift.
CoolSculpting needs a fold of fat that the applicator can draw in. The cheeks and temples do not have that kind of fold, and neither does the skin around the eyes. Their curves are also too tight for the applicator to sit flat. Cheek fullness also comes from deeper fat pads and from facial structure, which a surface treatment cannot reach.
Many patients find the opposite is true for the cheeks. As we age, the cheeks lose volume, and that loss is part of what lets the jowls form below them. Restoring that support with cheek filler can sharpen the lower face more than removing fat would.
Every face plan at our NYC medspa on Fifth Avenue starts with a close look at what is causing the fullness. We use Aura AI 3D imaging to map the skin and contours of the face from every angle, so we can show you where fat sits and where the skin has loosened. That tells us whether CoolSculpting, a skin tightening treatment or a mix of the two will give you the jawline you want.
When fat is the cause, we place the applicator under the chin, along the jaw or both. When laxity is the cause, we recommend a tightening treatment instead and tell you plainly that freezing fat would not help.

There is no downtime. The area may look a little red or swollen for a few hours, and it can feel tender or numb for a few days. These effects fade on their own, and you can go about your day right after the session.
Keeping your weight steady protects the result. CoolSculpting removes the treated fat cells, but the fat cells that remain can still grow if you gain weight.
CoolSculpting is a good fit for fat under the chin and along the jawline. It is the wrong tool for the cheeks and for jowls that come from loose skin, where skin tightening or filler does the work. The best way to know which applies to you is an in-person assessment, where we can check the area and build a plan around your goals.
